What is Neurosurgery

WHAT IS NEUROSURGERY

Neurosurgery is a surgical super-speciality in Health Care System. It deals with diseases or disorders of brain and spinal cord which need treatment in the form of surgery. As brain and spinal cord has a very intricate and complicated structure and function, surgeries of the brain and spine are also quite challenging. With all modern technological advances, surgical treatment has become highly specialised and very successful in treating majority of the central nervous system ailments.

IT'S A TEAM OF NEURO SPECIALISTS

We the team consists of following different specialists who spent at least two decades of dedicated
service in this speciality.

1. Neurosurgeon is the doctor who actually performs the brain & spine operation with the help of his assistants and entire team in the operation theatre.

2. Neuro Anaesthetists are the specialists who gives anaesthesia to the patients before they undergo brain operation. During surgery anaesthetist monitor and keep the patient under smooth anaesthesia and vigilantly keep a close watch on patient's vital parameters.

3. Neuroradiologist are the experts who opine and comment on the MRI , CT Images of the brain and spine. Their opinion in diagnosing the disease process based on imaging is of a prime importance. They also perform different endovascular procedures to treat vascular lesions in and around the brain and embolise feeders in certain types of brain tumours.

4. Neuropathologists examine the specimens of brain tumours and any other brain lesions in the laboratory. After staining, processing and studying the specimen they give their opinion/ verdict about the final diagnosis of the disorder, which decides the definitive treatment of the
disease. Recently a new dimension of assessment has been introduced that is the genetic and molecular evaluation of these brain tumours . This is done in a specialised lab which adds a lot of new information about brain and spinal tumours to predict the outcome and the future of these patients.

5. Neurologists are basically physicians who are specialised in assessing and diagnosing the brain and spinal diseases. To manage the patients with medication Is their responsibility. They are actively involved in the medical management of neuro patients.

6. Critical Care consultants are the doctors who look after the patients round the clock in the ICU(Intensive Care Unit). Most of the patients after surgery need ICU care. Patients sometimes need ventilatory assistance and very close monitoring of their vital function, their blood chemistry etc.,which is efficiently carried out by these specialists.

7. General Physician assess the patients before surgery and anaesthesia . Many preexisting illnesses complicate the management and it adds risk to patient's life. To put everything in order is sometimes a difficult task. The medical fitness for these neuro procedures is very important, all the health parameters have to be brought in proper order so as to reduce the risk during surgery.

WHAT MODERN GADGETS AND TECHNOLOGY WE USE ?

1) STEREOTAXY ( Stereotactic surgery of the brain)
This technological advance is as good as a robot in neurosurgery. This
requires CT scan and MRI image assistance. This apparatus helps to pin
point and reach to a very specific and deep areas of brain. This allows and makes it possible for a Surgeon to deal with tiny, small & deep seated lesions like tumours or inflammatory lesions in the brain.

2) CUSA(CAVITRON ULTRASOUND ASPIRATOR)
This gadget works based on ultrasound energy which helps in removing
solid brain and spine tumors very efficiently and quickly without disturbing the adjacent normal brain tissue. It vapourises and sucks the tumor tissue & significantly reduces surgical time duration.

3) OPERATING MICROSCOPE
Nowadays for majority of the neurosurgical procedures operating
microscope is used routinely. Microscope enhances and enlarges the view of operating field in narrow & deep areas of the brain with far better illumination and clarity for the surgeon. The magnified image or view helps the surgeon to restrict his movements and do a very precise and delicate work efficiently and confidently through a narrow passage.

4) NAVIGATION
This machine helps to decide the path to approach or plan the trajectory
(point of entry) for the exact location of the lesion in the brain. This tool is very useful to judge and assess the point of exact reach in the brain during the operation .

5) MRI (3 Tesla)
Magnetic Resonance Imaging(MRI) is the most commonly used imaging
modality apart from CT scan for brain and spine disorders. The biggest
advantage is that it does not emit radiation. Almost every small lesion or
abnormality in the brain and the spinal cord can be detected by this method. This machine has a strong magnetic field and it works on hydrogen atoms in the human body. The resolution and clarity of images give perfound information about normal and abnormal structures in the brain. MRI uses different sequences for detecting various brain and spine lesions.

WHAT ARE THE COMMON BRAIN AND SPINE SURGERIES ?

1. Brain and Spinal tumor removal (Neuro Oncho Surgery)

2. Repair of the skull fracture, dura and deformities of head (Head Trauma)

3. Removal of blood clots from the brain (Injury and Hypertension- Stroke)

4. Removal of pressure over the nerves in the spine (Decompressive Laminectomy)

5. Spinal fixation surgeries with prosthesis (Spondylolisthesis - Correction)

6. Removal of disc pressure over the nerves in the spine (Microdiscectomy)

7. Draining of the excess fluid from the brain (Hydrocephalus in children - Shunts)

8. Repair of the congenital birth defects of the head and spine in children (Craniosynostosis)

9. Pituitary tumor surgery through the nose and sphenoid sinus (Transnasal Endoscopic)

10. Intracranial aneurysm Coiling (Endovascular) or Clipping (Craniotomy)
